My "submit" button won't turn green.

 

In order to submit your Membership Commitment Statement, be sure to read the entire page and type out your signature exactly how it is displayed, including proper casing of your first and last name.

I am missing a checkbox on my form.

 

We have recently removed ALL of the checkboxes to make it easier for you to submit. Simply type out your signature exactly how it is displayed, including proper casing of your first and last name. The SUBMIT button will turn green and you can hit SUBMIT. If you do not see the updated page with all checkboxes removed then your device has cached the page. Please REFRESH your page and/or the entire browser. If that doesn't work, try a different browser (e.g., Firefox, Google Chrome, Safari, Internet Explorer, etc.) on your device. Then, navigate back to the Membership Commitment Statement and continue to fill it out.
